ARRIVED: 10a

BATH:  Sponge bath

DENTAL HYGIENE: Brushed  AM & PM

BREAKFAST: Cereal, blueberry scone & coffee

LUNCH: PB&J sandwich, apple slices, 7up

DINNER: Salad, meatloaf sliders, mac & cheese & 7-Up

DESSERT: None

EXERCISE: No

NAP: Yes

MEDS: AM meds with breakfast, PM Meds at bedtime, BS 166 – 2 units of insulin required

CHORES: Laundry (sheets, pad & PJs), made bed, helped Elena with cleaning, picked up mail, prepared meals

Elena – Cleaned

COMMENTS:  Elena took care of Kemper and Hillary before I arrived. I got Dr. Vesper up, he had a sponge bath and dressed before breakfast.  He took his morning meds with breakfast. Dr. Vesper did not want to get up this morning, he made it through breakfast and was awake for a very short time before going back to sleep and sleeping the entire day. He didn’t want to get up for dinner until I finally forced the issue at 7p. Naturally, he was wide awake and ready to sit up and watch TV after dinner. I let him sit up a little later than normal, since I still had laundry to finish. I did the dishes and set the table for morning breakfast and a.m. meds. Dr. Vesper took his p.m. meds and prepared for bed.

BREAKFAST: Cereal, mixed berry scone & coffee

LUNCH: Glucerna (after dental appointment)

DINNER: Potato leek soup, grilled cheese sandwich & 7-Up

DESSERT: None

EXERCISE: No

NAP: No

MEDS: AM meds with breakfast, PM Meds at bedtime, BS within range

CHORES:  Swept front entrance, cleaned glass doors, vacuumed floors, sanitized kitchen counters, laundry (sheets)

sanitized kitchen counters, touched up bathrooms, made bed, picked up garbage cans and mail, prepared meals

VISITORS/WORKERS: None

COMMENTS: Dr. Vesper was up at 10:30, had his morning meds with breakfast. After breakfast I assisted him with shaving, showering and dressing. We left at 1:30 for his appointment with Dr. Seibert, It was a double  appointment, cleaning, x-rays and a temporary crown. We didn’t get home until almost 5p. He was hungry (missed lunch) but his mouth was still very numb, I gave him a Glucerna to hold him over. I waited for the numbness to dissipate before giving him a soft dinner.  Dr. Vesper watched TV until bedtime. I cleared the table, did the dishes and set the table with AM meds and breakfast for tomorrow. He took his p.m. meds and I helped him change into PJs and get into bed.

NOTE: In addition to the dental work that was postponed from last summer, he had lost another crown. Dr. Seibert had to prioritize and do what was necessary to make him comfortable until they could schedule him back in next week.

LUNCH: Roast beef & Swiss sandwich, kiwi, melon & 7up

DINNER: Salad, baked tilapia, wild rice, peas, roll & 7-Up

DESSERT: None

EXERCISE: Yes

NAP: Yes

MEDS:  AM meds with breakfast, PM Meds at bedtime, BS – 164 / 2 units of insulin required

CHORES: Filled food & water bowls for Hillary & Kemper, emptied litter box, swept front entrance, cleaned glass doors, vacuumed, laundry (top sheet) watered plants, cleaned out refrigerator, dropped off garbage cans & recycling, picked up mail, prepared meals

VISITORS/WORKERS: None

COMMENTS: I fed Hillary and Kemper, filled water bowls, emptied litter box. I got Dr. Vesper up, he had a sponge bath and got dressed before breakfast. He took his AM meds with breakfast. In the afternoon, he had lunch, watched TV, exercised and took a nap. After dinner he watched TV while I did the dishes and cleaned the kitchen. I prepared morning meds and breakfast setup for tomorrow. Dr. Vesper took his pm meds and prepared for bed.

NOTE: Kemper wasn’t feeling up to par today, no diarrhea or vomiting, just a warm nose and very lethargic, he slept most of the day.

5:00-7:00     When I arrived, Dr. Vesper was relaxing in his recliner.  While he perused his mail, Stephanie brought me up to speed on the day’s events.  After she left, I chatted with Dr. Vesper, took his dinner order, and began cooking.  He received a call from his sister, Mary, and enjoyed a nice conversation with her till dinner was served.  His blood glucose level prior to eating was 152.  2 units of insulin were administered.

7:00-9:00     Dr. Vesper ate well at the beginning of his meal, but then slowed down.  He said he enjoyed the food and declined several offers to reheat it.  He repositioned his chair several times and I suggested he might be more comfortable in the living room.  He declined that as well, stating he wanted to finish his meal and remain at the table.  As we approached the end of my shift, I frequently reminded him that it was approaching bed time and there would be no time to watch TV from his recliner.  When he finally finished eating and agreed to get up, I realized he needed a clothing change.  We went to the bathroom, got into his pajamas, and completed his night time toilette.  He was highly fatigued and requested a ride in the wheelchair.  He was unhappy that I was taking him to the bedroom rather than to the living room and I apologized that I could not accommodate his request to stay up. (It was after 9:00)  He was happier when I offered to turn on his bedroom TV.   I then tucked him in and said good night.  I believe I heard him turn the TV off as I was leaving.  He did receive his night time medication prior to getting in bed.

Dinner:     Salad, petite sirloin steak, mashed potatoes, peas, naan bread, and diet cherry 7up.

VISITORS/WORKERS: Mossy- installed new fuse for septic pump
Clermont County Public Health – inspected septic system

COMMENTS: Dr. Vesper was awake and ready to get up when I checked on him around 10:30 this morning. Got him into the bathroom, spongebath, changed depend and clothes. He ate breakfast and took a.m. meds. He had lunch, watched TV, did exercise. He was wide awake today. No napping at all. Very alert today.

5:00-7:00     When I arrived, Dr. Vesper was relaxing in his recliner watching TV.  Stephanie brought me up to speed on the day’s events before she left.  I chatted with Dr. Vesper and took his dinner order.  Though generally quiet, he was much less fatigued than the day before.  When it was time for dinner, he came to the table unassisted and ate well.

7:00-9:00     The remainder of my shift ran in quiet, routine fashion.  After dinner, Dr. Vesper returned to the living room while I cleaned the kitchen, washed the dishes, and set out breakfast items and morning medications.  He made one trip to the bathroom without my prompting and no clothing change necessary.  He fell asleep in his recliner until bedtime.  I woke him, gave him his evening medication, and assisted him in the bathroom with his night time toilette.  Once in bed I tucked him in and said good night.

Dinner:     Salad, meat loaf, macaroni and cheese, green beans, garlic bread, and diet cherry 7up.
