5:00-7:00     When I arrived, Dr. Vesper was in the living room relaxing in his recliner.  After Stephanie left, we chatted for a few minutes, I took his dinner order, and began cooking.  He was very fatigued after his outing and slept soundly until dinner was ready.  He ate fairly well, but did not have the appetite I anticipated given his light lunch.  His blood glucose level prior to eating was 145.

7:00-9:00     The remainder of my shift ran in quiet, routine fashion.  After dinner, Dr. Vesper returned to the living room while I cleaned the kitchen, washed the dishes, set out breakfast items and morning medications, and took out trash.  At bedtime I gave him his evening medications, then assisted him in the bathroom with his night time toilette.  He was able to ambulate to his room where I helped him into bed, tucked him in, and said good night.

Dinner:     Salad, lemon pepper baked tilapia, wild rice, peas, toaster corn bread round, and diet cherry 7up.  No dessert.

Visitors:     None

Mon Feb 10/10-5

ARRIVED: 10am

CHORES: fed Hillary and Kemper, cleaned cat litter, swept bathroom, made bed, swept and vacuumed floors, sanitized counters

VISITORS: Mossy -took Dr. Vesper to dr

EXERCISES: yes

BLOOD GLUCOSE: n/a

BREAKFAST: cereal, blueberry scone and coffee

LUNCH: ensure

COMMENTS: I arrived at 10. Went and emptied urinal to see if we could get a fresh sample. Took care of Kemper and Hillary. Done chores. Got Dr. Vesper up at 10:30. He was dry but wet up the back (think it was sweat, again). To bathroom, spongebath, clothes and depend changed. Had breakfast. Mossy arrived to take Dr. Vesper to the Dr. They left about 12:15 and returned at 2:45. Dr. Vesper to bathroom and finally gave us a urine sample. He then had an ensure and rested the rest of the afternoon.

ARRIVED: 10am

CHORES: fed Hillary and Kemper, cleaned cat litter, swept bathroom, made bed, swept and vacuumed floors, sanitized counters

EXERCISES: yes

BLOOD GLUCOSE: 162/2 units insulin

BREAKFAST: cereal, blueberry scone and coffee

LUNCH: 1/2 grilled ham, turkey and cheese, grapes, yogurt

COMMENTS: I arrived at 10. Took care of Kemper and Hillary. Done chores. Got Dr. Vesper up at 10:30. He was dry but wet up the back (think it was sweat). To bathroom, spongebath, clothes and depend changed. Had breakfast, exercises, lunch, rested.

5:00-7:00     When I arrived, Dr. Vesper was relaxing in the living room watching TV.  Stephanie and I spent a few minutes discussing his day before she left.  I chatted with Dr. Vesper, took his dinner order, and began cooking.  When it was time to eat, Dr. Vesper elected to use the bathroom before coming to the table.  I helped him change his brief, but his clothing was dry.  He was able to use his walker there and then back to the table where he enjoyed a fairly healthy appetite.

7:00-9:00     After dinner, Dr. Vesper returned to the living while I cleaned the kitchen, washed the dishes, and set out breakfast items and morning medications.  At bedtime I gave him his evening medications, then assisted him to the bathroom for his night time toilette.  He made it to bed easily where I tucked him and said good night.

Dinner:     Salad, meat loaf, mashed potatoes, peas, garlic bread, and diet cherry 7up.

Visitors:     None

Note:     Dr. Vesper has not worn his lower orthodontic retainer the last two nights.  He has difficulty getting it in and says it’s uncomfortable.  He may need an adjustment due to the recent seating of his new crown.  I wanted to get a better look, but I think he was unaccustomed to me being in my dental hygienist mode.

5:00-7:00     When I arrived, Dr. Vesper was in his recliner watching TV.  As always, Michele brought me up to speed on the day’s events before she left.  It was a quiet start to my shift and that trend continued through the evening.  I chatted with Doctor for a few minutes, took his dinner order, and began cooking.  When it was time to come to the table he elected to use his walker and did so the rest of the time.  He enjoyed a hearty appetite and ate well.  Doctor’s blood glucose level prior to eating was 142.

7:00-9:00     After dinner, Dr. Vesper returned to the living room while I cleaned the kitchen and microwave, washed the dishes, and set out breakfast items and morning medications.  At one point he asked me to turn off the TV.   I was unable to engage him in conversation and offered some reading material.   I am unsure if he was bored or fatigued.  He closed his eyes and dozed lightly.  At bedtime I gave him his night time medication and assisted him in the bathroom with his evening toilette.  He was wet through his pajama bottoms, but clean up was quick and he ambulated well back to the bedroom.  I tucked him in and said good night.

Dinner:     Salad, sweet and sour chicken, wild rice, dinner roll, and diet cherry 7up.

Saturday, February 8, 2020 10am-5pm

ARRIVED: 10am

CHORES: fed Hillary and Kemper. cleaned cat litter, swept bathroom, refilled water pitcher and coffee basket for tomorrow, made bed, washed a few dishes, wiped counter tops. Set the table and made Dr. Vesper a salad to serve with dinner.

VISITORS: Jeanne to do the books and her husband the driveway.

EXERCISES: yes

BLOOD GLUCOSE: n/a

BREAKFAST: cereal, blueberry scone and coffee

LUNCH: cheese with nuts and raisins, water.

COMMENTS: I went in at 10:50 a.m. To wake Dr. Vesper up. He said he wanted to rest a little longer. So I told him I’d be back in about 20 minutes. I returned at about 11:20. I said, ok. It’s time to get up. Mossy just called. She wants you up and he said well Mossy is not here today. So I want another 30 minutes 🙂 He wasn’t actually sleeping either time I went in. He was just laying there with his eyes open. So I decided to allow him the extra time because it didn’t matter if he was lying with his eyes open in the bed or in his recliner. Finally at 11:50. He agreed to get up but choose to use the wheel chair this morning. After breakfast, into the LR. He is doing well today, alert, not sleepy but his urinal was only about 1/8 of the way full and his brief was completely dry. At his afternoon restroom break he was going to walk but before even getting off the LR carpet he decided to ride.

LUNCH: PB&J sandwich, apple slices & 7-Up

DINNER: Salad, ham steak, w/ Pineapple, steamed carrots, rolls & 7up

DESSERT: None

EXERCISE: Yes

NAP: Yes

MEDS: AM meds with breakfast, PM Meds at bedtime, BS was within normal range, no insulin required

CHORES: Filled food and water bowls for Hillary and Kemper, cleaned liter box, swept entrance, cleaned glass doors, vacuumed, sanitized counters, laundry (top sheet & towels) made  bed, watered house plants, filled bird feeders, & prepared meals

VISITORS/WORKERS:
None

COMMENTS:  I came in a little early this morning, in order to get a urine sample from Dr. Vesper. His urine has been very dark and infrequent this week. I talked to Dr. Wilson’s office yesterday and they thought they should do a urine analysis. I stopped this morning and picked up a specimen cup and came straight here to get a sample and take it back to the office. after finding glucose in the urine, Dr. Wilson decided he needed another specimen on Monday and a personal exam. When I arrived back at the house, I fed the animals and then got Dr. Vesper up, he ate breakfast and took a.m. meds. After breakfast I gave Dr. Vesper a complete sponge bath and helped him get dressed. He had lunch, watched TV, did exercise and napped while I finished chores and did laundry. After dinner, he watched TV until bedtime. I set the table for breakfast and morning meds. He took his p.m. meds and got ready for bed.
