Comments: Upon arrival, Dr. Vesper was watching television and Rose was finishing her 4:30 pm bathroom break. Rose came out of the bathroom to sit with me in the kitchen. Rose was very tired since the time I arrived. She seemed to have a hard time staying awake all evening. I made dinner. Dr. Vesper and I had to wake Rose to eat dinner. Rose ate well. I gave Rose her Targretin medication after dinner. Rose sat with Dr. Vesper after dinner while I cleaned up dinner dishes and the kitchen. Rose fell asleep again. I prepared medications and set things up for the morning. I gave Rose her 10 pm medications and assisted her with getting ready for bed. I tried to keep Rose awake, but she just kept falling asleep.

Sunday, December 6, 2015/ 1:30 pm – 9:36 pm

Arrived: 1:30 pm

Dental hygiene: brushed and flossed pm

Lunch: cottage cheese with pineapple

Dinner: salad, salmon patties, asparagus, kefir milk/7 UP

Dessert: ice cream and decaf. coffee

Exercise: yes

Medicines: 2 chewable vitamins, tums, 2 pm medications,  Targretin with dinner, 1/2 Xanax, 10 pm medications, eye drops at bedtime.

Fluid Intake: 12 oz water and 16 oz kefir milk with lunch and dinner

Urine/BM Output: urine and normal BM at 4:30 pm and urine and large BM at 8:30

Chores: sanitized kitchen countertops, prepared lunch and dinner, washed lunch and dinner dishes, and cleaned up kitchen

Visitors/Workers: Mossy from Home Instead

Comments: Upon arrival, Mossy updated me on everything. Dr. Vesper and Rose were sitting in the living room watching television. I sanitized countertops. Then, I made Rose lunch. She at well. I gave Rose her 2 pm medications. Rose was in very good spirits. I worked with Rose during exercise. Then, I took Rose to watch television with Dr. Vesper. I propped Rose’s feet up at 3 pm. Rose napped for a little while. Kemper went out at 2:30 pm and came back with a deer leg. After Kemper had his food, he went out and came back with another bone from a deer. All bones were wrapped and thrown away. At 4:30 pm, I took Rose for her bathroom break. I started dinner at 5:15 pm. The Vespers ate dinner at 6:00 pm. Rose at very well. I gave Rose her Targretin. Then, I clean up the kitchen, prepared 10 pm medication and tomorrow mornings medication. At 8 pm, I gave Rose her 10 pm medication. I took Rose at 8:30 pm to get her ready for bed. She used the bathroom a lot. I put her in bed with her eye drops. Toilet overflowed, but it was corrected.

Friday, December 4, 2015/ 5 pm – 9 pm

Dessert: kiwi lime pie

Medications: Targretin w/dinner, 1/2 Xanax, 10 pm medications, eye drops at bedtime

Fluid Intake: 8 oz kefir milk with dinner

Urine/BM Output: 8:40 pm urine and large, loose BM in brief

Chores: sanitized kitchen counter tops, prepared dinner, washed dishes, and cleaned up kitchen

Visitors: Mossy from Home Instead

Commets: Mossy updated me upon arrival to the Vesper’s home. Dr. Vesper and Rose were sitting in the living room watching television.  I started dinner at 5:40 pm. Dinner was served at 6:30 pm. Rose was in good spirits. She ate well. After dinner I gave rose her medication (Targretin). I washed the dishes, prepared medications and for tomorrow. I gave Rose her 10 pm medications. Then, I took Rose back to get her ready for bed at 8:40 pm. I cleaned Rose up, put her night clothes on her, brushed her teeth, put her in bed, and put eye drops in her eyes at 9:15 pm. Rose was alert all evening.

Monday, November 23, 2015/ 5 pm – 9 pm

Dental Hygiene: Brushed pm (Rose would not open her mouth to floss.)

Dinner: LaRosa’s Pepperoni Pizza, kefir milk/7UP

Dessert: ice cream and decaf. coffee

Medications: Targretin with dinner, 1/2 Xanax, 10 pm medications, eye drops at bedtime

Visitors/Workers: Roberta from Home Instead, Jennifer (daughter) and kids

Comments: Upon arrival, Roberta updated me. Dr. Vesper and Rose were sitting in the living room watching television. I covered Rose as her legs were cold (I have noticed in the evenings when getting Rose ready for bed that her legs are very cold). Jennifer and the kids showed up shortly after I arrived. Jennifer went out to the store and picked up food. Jennifer provided LaRosa’s Pizza. I helped Rose eat. She ate well. Rose was in good spirits. She said she was doing good. I noticed a mark on Rose’s bottom lip at dinner time. I thought then she must have gotten something on her lip. I gave Rose her medication after dinner. I cleaned up the kitchen and prepared for tomorrow. I gave Rose her 10 pm medication and helped her get ready for bed. I took Rose in the bathroom to brush and floss her teeth. This is when I noticed the spot on Rose’s lip looked more like a bruise. I took care of Rose’s restroom break and finished putting her night clothes on her. Then, I put Rose in bed and put eye  drops in her eyes. I told her good night. She said ok.

Medication: Targretin with dinner, 1/2 Xanax, 10 pm medications, eye drops at bedtime

Fluid Intake: 8 oz kefir milk with dinner

Urine/BM Output: 8:30 pm urine and small BM in brief only

Chores: sanitized kitchen countertops, prepared dinner, washed dishes and cleaned up kitchen

Comments: Upon arrival, Dr. Vesper and Rose were watching television. Mossy instructed me with what to fix for dinner. I prepared dinner and helped Rose to eat. Rose ate very well. Rose was in good spirits. I gave Rose her after dinner medication. I cleaned up the kitchen and prepared everything for Rose and Dr. Vesper. I gave Rose her 10 pm medication. Then, took Rose to the restroom to get her teeth brushed and flossed and new clothes for bed. Rose was in bed by 9:00 pm. I put eye drops in Rose’s eyes. I told her good night. She said okay.

Saturday, November 21, 2015/ 5 pm – 9 pm

Medications: Targretin with dinner, 1/2 Xanax, 10 pm medications, eye drops at bedtime

Fluid Intake: 8 oz kefir milk with dinner

Urine/BM Output: 8:30 pm urine and small BM

Chores: sanitized kitchen counter tops, prepared dinner,  washed dishes, and cleaned up kitchen

Visitors/Workers: Rhonda from Home Instead

Comments: Upon arriving, Rose was sitting in the kitchen with Rhonda. Rhonda updated me. Dr. Vesper was at church. I made dinner while Rose sat with me in the kitchen. Rose was very alert. Dr. Vesper arrived home at 6:17 pm. I served dinner a few minutes later. Rose ate well. I gave Rose her medication at dinner. After dinner, Rose sat with Dr. Vesper in the living room and watched television. I cleaned up the kitchen, prepared and gave 10 pm medication, prepared tomorrow morning medication, and took Rose to the restroom. I prepared Rose for bed and put her in bed at 8:55 pm. I put drops in her eyes.

Comments: Upon arrival, Mossy was finishing up chores. Rose and Dr. Vesper were sitting in the living room watching tv. I made dinner. Helped Rose with eating. She was taking an extra long time eating. Rose was in good spirits. I gave Rose her medicine with dinner. I cleaned up. I gave Rose her 10 pm medication. Prepared Rose for bed by putting her night shirt and pants on with a new brief and bushing her teeth. I put drops in her eyes and covered her.
