Comments: Upon arrival, Dr. Vesper was sitting at the table finishing breakfast. Rose was sleeping. At 10:30 am, I reminded Dr. Vesper of his doctor appointment. I woke Rose at 10:35 am. I assisted Rose out of bed, to the bathroom, dressed, and hair brushed. Rose did not look like she felt well. I prepared breakfast. Rose ate well. I gave her morning medications. At 12:30 pm, I assisted Rose to the bathroom. I prepared lunch. Rose took her 2 pm medications. She ate well. Rose napped off and on all day.

Saturday, February 13, 2016/ 10 am – 5 pm

Arrived: 10:00 am

Dental Hygiene: brushed and flossed pm

Breakfast: omelet with vegetables, (2) pieces of bacon, (1) toast, and orange juice with Miralax

Lunch: 1/2 ham sandwich, cottage cheese with pineapple, kefir milk, and water

Medications: Levothyroxine, Miralax in orange juice, Targretin with breakfast, (1) tums, (2) multivitamins, 2 pm medications

Nap: yes

Exercise: yes

Fluid Intake: 8 oz orange juice, 8 oz kefir milk, 32 oz water

Urine/BM Output: 10:30 am urine, 12:30 urine, 4:30 urine and small BM

Chores: sanitized kitchen countertops, prepared breakfast and lunch, cleaned up kitchen after breakfast and lunch, watered plants, made bed, vacuumed living room

Comments: Upon arrival, Dr. Vesper and Rose were both sleeping. From the time I arrived at 10 am until the time I assisted Rose out of bed, Rose sneezed about 10 times.  At 10:30 pm, I assisted Rose out of bed and to the bathroom for morning activities. I clean, dressed, and fixed Rose’s hair. I prepared breakfast. Rose ate well. Rose was in a good mood. I gave Rose her morning medications. After cleaning up the kitchen, I assisted Rose to the bathroom at 12:30 pm. Rose assisted me with making the bed. She did a very good job. Rose continued to sneeze during the day. I suspected she has a cold. I prepared lunch for Rose. I gave Rose her 2 pm medications. I assisted Rose with exercise. Dr. Vesper did  not wake until 2:30 pm. At 4:30 pm, I assisted Rose with the bathroom. Rose’s nose started to run at 5 pm.

Monday, February 1, 2016/ 10 am to 5 pm

Arrived: 10:00 am

Dental Hygiene: brushed and flossed pm

Breakfast: orange juice with Miralax, oat meal, (2) pieces of bacon, (1) piece of toast, water

Lunch: 1/2 ham sandwich, cottage cheese with pineapple, 1/2 blueberry muffin, Kefir milk, water

Exercise: yes

Nap: yes

Medications: Levothyrozine, Miralax with orange juice, Targretin with breakfast, (1) tums, (2) multivitamins, 2 pm medications

Fluid Intake: 8 oz orange juice with Miralax, 8 oz Kefir milk, 32 oz water

Urine/BM Output: 10:30 urine only, 12:30 nothing, 4:30 urine and very, very small, hard BM

Chores: sanitized kitchen counter tops, prepared breakfast and lunch, cleaned up breakfast and lunch, vacuumed living room, watered plants, made bed, walked Kemper

Visitors/Workers: Susan from Home Instead and Mark from Glass company

Comments: Upon arrival, I let Mark in to measure the glass in the attic. Dr. Vesper and Rose were sleeping. I started preparations for breakfast. At 10:30 am, I gave Rose her medicine, helped her get up out of bed, and into the bathroom. I helped dress and brushed her hair. Then, Rose helped me make breakfast. I assisted Rose with breakfast. She ate well. I gave Rose Targretin with breakfast. At 12:30, I assisted Rose in bathroom and brushed and flossed her teeth. Rose was in good spirits. At 2 pm, Rose helped make lunch. I gave Rose tums and vitamins. I assisted Rose with lunch. Then, I gave Rose the 2 pm medication. At 4:30 pm, I took Rose to the bathroom. She is very constipated. Rose is having a difficult time with BM. Otherwise, Rose did very well today

Saturday, January 30, 2016/ 10 am – 5 pm

Arrived: 10:00 am

Breakfast: omelet with vegetables, (2) bacon, (1) toast, orange juice with Miralax, and water

Medications: Levothyroxine at 10:30 am, Miralax in orange juice, Targretin with breakfast, 2 pm medications, (1) tums, and (2) vitamins

Urine/ BM Output: 10:30 am urine and medium, normal BM, 12:30 pm urine and medium, normal BM, 4:30 urine and medium, normal BM (Rose also had a BM that she said “she can’t”. She was unable to pass at this time.)

Comments: Upon arrival, Dr. Vesper and Rose were asleep. I prepared breakfast and cleaned up Dr. Vesper’s breakfast. At 10:30 am, I helped Rose up out of bed. I gave her the Levothyroxine. I assisted her with bathroom needs, dressing, and brushing hair. I finished breakfast and assisted Rose with her meal. Rose ate well. I gave Rose the Targretin with breakfast. Rose was in good spirits, but she was rather sleepy. I assisted Rose with bathroom needs. I cleaned up the kitchen. At 2 pm, I made lunch and assisted Rose with her lunch. I gave Rose her 2 pm medicine. I helped Rose with exercises. I propped Rose’s feet up next to Dr. Vesper. As I was giving Rose a drink, Rose said to me “Do you want to play?”. I cleaned up the kitchen from lunch, watered the plants, and vacuumed the floor. Dr.  Vesper laid down for a nap at 3:30 pm. I woke him at 4 pm to let him know about getting ready for church. I woke him again at 4:15 pm about church and taking Kemper for a walk. At 4:30 pm, I took Rose back to the bathroom and told Dr. Vesper it was 4:30 pm. Dr. Vesper got up and left for church. I assist Rose with bathroom needs. Rose said she was doing well today. Rose dosed off a lot today.

Lunch: cheesy potatoes with ham, cottage cheese with pineapple, kefir milk, water

Medicines: Levothroxine, Targretin, 2 pm medications, (1) tums, (2) multivitamins, Miralax in orange juice

Fluid Intake: 32 oz water, 8oz orange juice, 8 oz kefir milk

Urine/BM Output: 10:30 urine only in brief, 12:30 urine only, 4:30 urine and large, firm BM (urine in brief)

Chores: sanitized kitchen counter tops, prepared breakfast and lunch, washed dishes and cleaned up kitchen, watered plants, made bed, vacuumed living room

Comments: Upon arrival, the door was locked. Dr. Vesper had not been up. Once I got in, I started preparing breakfast for Rose. At 10:30, I gave Rose medicine and helped her up out of bed and into the bathroom for toiletries. I finished breakfast and assisted Rose with her breakfast. I gave her Targretin with breakfast. Rose was in a more quiet mood today, but she like to make noises with her mouth and laugh about it. I took her over to the window and let her look out for a while. She enjoyed this. At 12:30 and 4:30, I took her for more toiletries. At 2:00, Rose helped me prepare lunch. I assisted Rose with lunch. She ate well.

 

 

Sunday, January 17, 2016/ 5 pm – 9 pm

Arrived: 5:00 pm

Dinner: crab cakes, peas, kefir milk/ 7 UP

Dessert: ice cream

Medications: Targretin with dinner, 1/2 Xanax, 10 pm medications, eye drops at bedtime

Fluid Intake: 8 oz kefir milk with dinner

Urine/BM Output: 8:30 pm urine and very small BM

Chores: sanitized kitchen counter tops, prepared dinner, washed dishes and cleaned up kitchen

Visitors/Workers: Mossy from Home Instead

Comments: Upon arrival, Dr. Vesper was watching television. Rose was with Mossy in the bathroom. When Rose came out of the bathroom, she seemed to be doing well. I started dinner with Rose’s help. I assisted Rose with dinner. I gave Rose Targretin. Rose sat with Dr. Vesper and watched television. I cleaned up the kitchen and washed the dishes. I prepared everything for tomorrow. I gave Rose the 10 pm medications and prepared her for bed. Rose told me she did not want to go to bed a couple of times. We took care of toiletries. I put Rose in bed and administered eye drops.

Saturday, January 16, 2016/ 10 am – 5 pm

Arrived: 10:00 am

Dental Hygiene: brushed and flossed PM (Rose and Dr. Vesper woke up last, so brushing and flossing did not occur in the morning.)

Breakfast: omelet with vegetables, 2 pieces of bacon, a piece of toast, orange juice with Miralax, and water

Lunch: greek yogurt with fresh pear, kefir milk, and water

Exercise: no

Nap: no

Medications: Levothroxine, orange juice with Miralax, (3) Targretin with breakfast, 2 pm medications, (1) tums, (2) mulitivitamin

Fluid Intake: 32 oz water, 8 oz orange juice, and 8 oz kefir milk

Urine/BM Output: 11::30 am urine only in brief and 4:00 urine and very small, loose BM

Chores: sanitized kitchen counter tops, prepared breakfast and lunch, washed dishes and cleaned up, watered plants, vacuumed, made Dr. Vesper and Rose bed

Visitors: Mossy and Roberta from Home Instead

Comments: Upon arrival, both Dr. Vesper and Rose were sleeping. At 10:30 am, I checked on Rose to determine if she was ready to get up. Dr. Vesper wanted some more time. At. 11 am, I checked on both of them again. Dr. Vesper said Rose was ready to get up. We talked for a little and I proceeded to get Rose up. I gave Rose her medicine. Rose was in good spirits. Rose to restroom at 11:30 am. I prepared breakfast and assisted Rose with her breakfast. Rose ate well. I gave Rose the Targretin with breakfast. I proceeded with daily chores. At 2 pm, I made lunch. I gave Rose tums and multivitamins. I assisted Rose with lunch. She ate well. I gave Rose the 2 pm medications. I propped Rose’s feet up in the living room. Rose did not take a nap. She complained quite a bit. I tried to determine the problem, but I could not determine anything. However, I have noticed she was leaning to the side quite a lot. So, Rose was against the bar of the wheel chair. I put a pillow between her and the bar on the wheel chair. I took her to the restroom at 4 pm due to having missed the 12:30 pm time. Rose was in a good mood. She was making clicking noises and she laughed about it.
