Arrived at 2 Rose was napping .Woke her and gave her 2:00 meds and fed her lunch which was stew and Kefir milk. Brushed teeth after lunch and then napped while be sitting in the living room next to Dr Vesper refused anything to eat
4:00 Fed Kemper
4:15 toilted Rose ,she had loose stool and urine Her pants had BM smear so I rinsed them out and put them on the washer. Rose went back to the living room to continue napping
Dr Vesper toilted himself twice on this shift

November 13,2017 Jan Lammert

Arrived at11am Filling in for Michelle. Andrew greeted me and gave me a report on the morning
Gave Rose her levoroxine and eye drops then toilted her and gave her a sponge bath dressed her and did her ROM exercises
Breakfast bacon, toast with orange marmalade, oatmeal and peaches, orange juice with Miralax and morning meds
Urine brief and pjs urine soaked 2:15 toiletng changed pad, urine
Brushed and flossed after meals
Lunch 1/2 ham and cheese ,grapes and banana , Kefir milk
Dr Vesper ate a 1/2 ham and cheese and 7up
Cleaned litter box, counter and did dishes
The nurse came for Dr Vesper and took his sugar and vitals She said everything was good and she would call Mossy
Dr Vesper had me get his mail and looked at his mail and watched TV (He watched the noon news 5times while I was here) He toilted himself 2x’s on this shift I noticed his soiled breif hanging from his walker I threw them away.
Kroger Pharmacy called and said there is a prescription ready
Rose began napping at 3:15

Arrival Stephanie was at the houseand showed me how to use the glucose monitor I gave Rose her eye drops and Levothyroxine Toilted her and gave her a sponge bath and dressed her
Breakfast steel cut oats ,oranges, bacon,toast, orange juice
Lunch Chicken, fruit, cottage cheese and, Kerflix milk
BM and urine. Overnight depends soaked,12:30 small bm,4:15 small bm
Meds morning meds eye drops, and 2:00pm meds with Tums and Vitamin
Brushed and flossed After meals
Exercise ROM
Naps Both Dr.andRose napped for about 45 minutes
Chores cleaned cat box and fed cat, cleaned front door window,made bed,
Comments There was a problem with the pool closing so I had to call Mossy.
Dr Vesper I couldn’t get enough blood for the glucose monitor and he didn’t want me to try anymore. He was able to transfer with assist and went to the bathroom once and had a large BM. He had a ham sandwich and cherry 7up for lunch. Napped on and off

Bath. Sponge bath
Breakfast. Steel cut oatmeal, toast with ginger jam ,bacon, orange juice and Miralax
Lunch Chicken , cottage cheese, fruit and Kerflix milk
Urine. Overnight depends soaked with urine 12:30 small bm 4:30
Meds. Morning meds,eye drops and 2:00 meds Tums and Vitamin
Excercise ROM
Brushed and flossed After meals
Naps off and on Both Rose and Dr Vesper had an afternoon nap
Chores. Cleaned cat box,fed cat fed dog ,did dishes, cleaned glass on front door
Comments Problem with pool cover had to call Mossy .Dr Vesper needed assistance to get in his recliner Refused toiletng at 12:30 Gave him his water to drink .I gave him a ham sandwich and some cherry 7up I attempted to get his blood sugar but after 3 tries I couldn’t get the monitor to read He said he didn’t want me to try anymore and would wait for the night aide Dr Vesper refused toileting at 3:00

5-8pm Made dinner ,clean counter and did dishes Dr Vesper got his eye drops and got the table ready for breakfast
Dinner Salmon ,salad,rice,peas and Rose had a piece of chocolate cake and protein milk Dr Vesper refused dessert
8pm Took Rose to the bathroom and changed her brief (urine) , gave her her night time meds, brushed her teeth and put her pajamas on Dr Vesper took his meds and eye drops .I took Rose to bed and gave her eye drops

Arrived at 10:00 Dr Vesper was eating breakfast I gave him his eye drops and asked if he was going to church He replied “Yes” I gave Rose her eye drops and toileted her and dressed her. While I was feeding her Andrew came to drop off the truck. I found out that Dr Vesper is not allowed to drive.Stephanie arrived and explained to Dr Vesper that he couldn’t drive yet and gave him his morning meds that he forgot to take

Bath. Sponge bath
Breakfast orange juice with Miralax,bacon,toast with butter and orange marmalade,cream of wheat
Lunch fruit and cottage cheese protein milk and crackers with pimento cheese
Urine/BM. overnight depends wet with BM smear 12:30 small BM and slight urine output
Meds morning meds and Levothyroxine and eye drops
Excercise ROM
Brushed / flossed after breakfast
Fluids water, orange juice and protein drink
Naps off and on through this shift
Chores. Made bed, cleaned cat box,swept front entry,wiped down counters
Comments Dr Vesper went back to bed after breakfast Dr Vesper refused lunch and stated that he was okay when I went in at 1:00@

Arrived at 10:00 and gave Dr Vesper his eye drops He went back to bed and at 11:00 went to church Fed the cat and cleaned the litter box Got Rose up at 10:30 and gave her eye drops and Levothyroxine
Bath. Sponge bath
Breakfast Bacon,toast,orange juice with Miralax ,cream of wheat with mango slices
Lunch. Ham and cheese sandwich,cantaloupe,grapes and protein drink
Urine/BM. Overnight depends wet 12:30 urine and medium bowel movement

Chores Made bed ,washed dishes,cleaned litter box,sanitized counters,swept entry,
